Please browse the list of membership types below. Once you're ready to become a member, please download the appropriate forms found on the state website (Forms), print, complete the forms, and send them directly to the state office.
Please use the basic membership application form if you are applying as an active member (those with a degree from an accredited university), active member on points (those without a music degree from an accredited university), provisional membership, student membership, or a business affiliate membership. Please remember to also download the checklist that corresponds with the active member form, and the student member form. If you are applying on the point system please also download the point system membership form and its corresponding checklist. If you are a member who would like to change the status of your membership, add an instrument or subject, transfer to our branch, or extend your provisional membership, please use the appropriate forms found on the state website (Main Membership Page).
